PERTH AMBOY — The City of Perth Amboy, Mayor Wilda Diaz, Office on Aging And the Middlesex County Office on Aging and Disabled Services Announce that the 2013 Senior Farmer’s Market Nutrition Program Voucher Applications will be available at the Alexander F. Jankowski Community Center, One Olive Street, Perth Amboy Starting May 21st by appointment only.

Seniors must meet income guidelines and must bring proof of income at the time of the appointment.
