Vaccination Program

NEW JERSEY – The New Jersey Department of Health has launched a program that can provide in-home COVID-19 vaccine appointments for homebound persons and has begun accepting requests for this important service.

If you are unable to leave home to receive a COVID-19 vaccine, or you are the family member or caregiver of someone who is homebound, you may request an in¬-home vaccination appointment by completing a Request for In-Home Vaccination at https://covid19.nj.gov/homeboundvax

For assistance completing the survey by phone, please call the NJ COVID-19 Vaccine Call Center at 1-855-568-0545.

After you submit the request, the Department of Health will share your information with a home health agency, local health department, or other vaccination provider, and you will be contacted to schedule an in-home vaccination appointment.

Walk-in Covid Vaccines

PERTH AMBOY – Beginning Monday, May 3, Hackensack Meridian Raritan Bay Medical Center Perth Amboy will be accepting walk in’s for the COVID-19 vaccine between 8:30 a.m.-3:45 p.m., Monday-Friday. Anyone interested in getting the COVID-19 vaccine can come to the front lobby of the hospital.

12-17 Years Old

PERTH AMBOY/SOUTH AMBOY – Now that clearance has been given to vaccinate persons between 12-17 years old with the Pfizer vaccine, the Raritan Bay Medical Center is offering walk in appointment for people in this age group.  The vaccine is available beginning Monday, May 17, 2021 at:

1. Raritan Bay Perth Amboy – Open to walk-ins as of Monday (5/17) M-F 8:30-3:45

2. Raritan Bay Old Bridge – Open to walk-ins as of Monday (5/17) M-F 8:30-3:45

Children being vaccinated MUST be accompanied by a parent or guardian who can sign the required consent forms.

PERTH AMBOY – Effective April 14, 2021, the City of Perth Amboy has launched a dedicated phone number and online registration for city residents only, ages 19 and older, to register for a COVID-19 vaccination appointment.

Atlantic County Megasite (Atlantic City Convention Center, 1 Convention Boulevard, Atlantic City): 9 am to 4 pm

Bergen County Megasite (Racetrack at Meadowlands, 1 Racetrack Drive, East Rutherford): 8 am to 12 pm

•Burlington County Megasite (Moorestown Mall, 400 NJ-38, Moorestown): 9 am to 5 pm

•Gloucester County Megasite (Rowan College of South Jersey, 1400 Tanyard Road, Sewell): 8 am to 12 pm and 2 pm to 6 pm

•Middlesex County Megasite (New Jersey Convention and Exposition Center, 97 Sunfield Avenue, Edison): 8 am to 12 pm

$6.5 Million for Vaccine Awareness

Funding Made Possible from Provision Pallone Authored in the American Rescue Plan

Washington, DC – Congressman Frank Pallone, Jr. (NJ-06) today announced that New Jersey will receive $6.5 million for COVID-19 vaccine awareness campaigns, education, and outreach. The funding will help expand outreach to increase vaccination rates and will be distributed through the Centers for Disease Control and Prevention (CDC).

“I’m glad to see funds from the American Rescue Plan are making their way to New Jersey to help increase our state’s COVID-19 vaccination rate,” Congressman Pallone said. “COVID-19 vaccines are the key to rebuilding our economy, protecting the health of our community, and safely returning to normal patterns of life. It’s critical that everyone in New Jersey and across the country know that these vaccines are safe, effective, and widely available. Everyone must do their part and get a vaccine, so we can end this devastating pandemic.”

Pallone’s Energy and Commerce Committee authored the provision in the American Rescue Plan that Congress passed and President Biden signed into law in March. The American Rescue Plan established a national vaccination program and provided critical funding and resources to increase COVID-19 vaccinations across the country
